Melatonin, (N-acetyl-5-methoxytryptamine) is a hormone (indole) produced primarily by the pineal gland in the brain. Nocturnal secretion of melatonin is at its highest during childhood, and then decreases with age. This may be the reason for insomnia in the elderly. Supplemental intake of melatonin has been shown to help regulate circadian rhythm in individuals whose sleep patterns have been disrupted by air travel, shift work or other circumstances.

Although most studied and proven for this vitally important sleep/wake regulatory role, the list of conditions for which melatonin may have benefit is very long. It appears to regulate a system of self-repair and regeneration. As this hardworking hormone diminishes with age, our biological functions are impaired. In addition to its hormone actions, melatonin also has strong antioxidant properties. Melatonin directly scavenges both hydroxyl and peroxyl free radicals, and it does so more effectively than most other antioxidants. It also greatly potentiates the efficiency of other endogenous and exogenous antioxidants. Melatonin is especially important for protecting cellular DNA against peroxynitrite damage by inhibiting peroxynitrite free radical reactions.

Melatonin’s mechanisms, including it anti-inflammatory effect, toxic free-radical scavenging, glutamate neurotoxicity protection, and others may be associated with headache pathophysiology in some ways. As is the case in the study of melatonin’s role in headache management, early studies look promising for melatonin’s role in healthy blood pressure, G.I. inflammation, smoking cessation, eye health, healthy mood and for normalizing sleep patterns disturbed by specific conditions. More studies are needed in these areas.
